Jennifer Martyn, Attorney, Guardianship, Downers Grove: Jennifer has been a licensed attorney for 27 years and practices in the areas of elder law and guardianship. She will discuss the legal steps to ensure that Powers of Attorney can be put into place to convey directives prior to an individual becoming incapacitated or being unable to convey said directives.
Susan Sinderson, Executive Director, Community Adult Day Center: Hear how an Adult Day Center can help family members support their loved one, and how a Day Center can enhance individuals' lives while helping them live in their own home for as long as possible. Kerry Quirin, Senior Living Advisor, Senior Living Experts: Kerry will talk about Senior Living options in the Chicagoland area including defining the levels of care, pricing, services, and amenities available.
Kerry Prichard, MSW and Lynette Cremerius, Hospice Specialist; UChicago AdventHealth St. Thomas Hospice: Kerry and Lynette will discuss how advance care planning is an ongoing process in which individuals explore and discuss their goals, values, and understanding of their health to guide future end-of-life treatment decisions. Participants will learn when and how to start end-of-life conversation and how to gather valuable information for future decision making.
